Below is the electronic health form. There is no more paper health form, as there was in years past. This form should be filled out after you complete the registration form. After filling out the health form, we will need a copy of your child’s immunization records and a letter of consent signed by your child’s doctor. That form should be scanned and emailed to director@southernmainestringcamp.org.
